PERSONAL DETAILS<\/h6>\n<p>Please use block capitals for this section of the form.<\/p>\n<ul>\n<li><strong>Title<\/strong><br \/>\nUse Professor, Dr, Mr, Mrs, Miss or Ms.<\/li>\n<li><strong>Degrees<\/strong><br \/>\nOnly certain degrees are recognised by ECBA for the purposes of the European Professional Biologist title; a list of these is given in Annex 2 of the Regulations.<\/li>\n<li><strong>National biological associations<\/strong><br \/>\nThis should be one of the ECBA member bodies listed in Annex 1 of the Regulations.<\/li>\n<\/ul>\n<h6>2. ADDRESS FOR CORRESPONDENCE<\/h6>\n<p>Please use block capitals for this section of the form.<\/p>\n<h6>3. EDUCATION AT UNIVERSITY LEVEL OR EQUIVALENT<\/h6>\n<ul>\n<li><strong>Title of degree<br \/>\n<\/strong>Write the title(s) of the degree(s) that you have received, eg Bachelor of Science (BSc)<\/li>\n<li><strong>Subject (and specialisms)<\/strong><br \/>\nWrite the subjects and specialisms as specified on your degree certificate.<\/li>\n<li><strong>Period of study<\/strong><br \/>\nWrite the year in which you started your degree course and the year in which you obtained the degree, eg 1972 &#8211; 1976. If the degree was obtained by part-time study, write &#8218;part-time&#8216;.<\/li>\n<li><strong>Professional titles<br \/>\n<\/strong>Write the name(s) of any professional or other qualification(s) you have received, eg Chartered Biologist (CBiol). Under &#8218;Date&#8216; write the year in which the qualification was awarded.<\/li>\n<\/ul>\n<h6>4. PROFESSIONAL EXPERIENCE<\/h6>\n<p>Professional experience may include time spent studying for a higher degree.<\/p>\n<ul>\n<li><strong>Your duties and responsibilities<\/strong><br \/>\nGive brief details of your main duties and responsibilities. Continue on another sheet of paper if necessary.<\/li>\n<li><strong>Knowledge and skills<\/strong><br \/>\nDon&#8217;t forget to attach a concise supporting statement giving examples of points 1 to 5.<\/li>\n<li><strong>Additional material<\/strong><br \/>\nPlease attach a complete list of your publications etc.<\/li>\n<\/ul>\n<h6>5. REFEREES<\/h6>\n<p>Please give the name and address of 2 referees who are familiar with your work.<\/p>\n<h6>6. DECLARATION<\/h6>\n<p>Don&#8217;t forget to sign and date the declaration.<\/p>\n<h6>7.
